This is for general information that in the advertisement for Grade-III and Grade-IV posts under the establishment of Directorate of Health Services (Family Welfare), Assam the eligibility Age of candidates for various categories should be read as :

Age limit : Candidate must not be less than 18 years and more than 44 years on 01.01.2018. The upper age limit is relaxable by 5 years for SC, ST(P) and ST(H), 3 years for OBC and 10 years for differently abled candidates.

Selection process for DHS FW Assam Grade III Recruitment

How to apply Director of Health Services (Family Welfare), Assam Grade- III Recruitment

1. Interested candidates shall have to register and fill up the online Application Form available at the official website of Directorate of Health Services (FW) Assam, https://dhsfw.assam.gov.in.
2. Online Portal for applying online for different posts will be available from 2nd November, 2018 onwards and shall close at 24:00 hrs of 16th November, 2018. Applicant may take a print of the final submission of Application form even after the closing date till the date of the examination.
3. Applicant must carefully choose the Post he/she applying for, as it cannot be changed later on. 
4. Before he/she proceeds to apply, he/she must ensure that he/she has read and understood the eligibility criteria cited in the advertisement & the instructions carefully for the post he/she is applying for. In case, he/she fail to upload or upload any false/incomplete document during the time of online application or fails to produce any relevant certificate/marksheet/affidavit or any other document relevant for the particular post during the time of document verification or at any stage; his/her candidature for the relevant post will stand rejected at any stage whatsoever.
5. For registration, applicant shall click on to ‘APPLY ONLINE’ button on the right hand panel to register. During initial registration process, Applicant must enter his/her valid E- mail ID, valid Mobile No. for OTP verification process. Applicant must generate a password for future login process.
6. After OTP verification, e-mail will be sent to the registered e-mail ID of the Applicant containing the username and password generated by the Applicant for future logins.

Note: Multiple Registrations is not allowed using the same e-mail ID and Mobile no.

7. Using the same username(E-mail ID) and password, registered applicants may Login for filling up of application form, editing of form, uploading of photo, signature and relevant documents. Applicant may login at any time to download and print the final application form till the issue of Admit Card.
8. Date of uploading of E-Admit Card, rejection list, accepted list shall be informed subsequently via website and local newspapers. Applicants are advised to keep track of the website and newspaper for any updates.
9. The following documents in original will have to be produced at the time of document verification. Scanned copies of the following should be uploaded in the online portal:

(a) Scanned copy of recent passport size colour photograph.
(b) Scanned copy of signature.

The following documents must be self attested and uploaded :

(c) HSLC Admit Card or Birth Certificate as proof of age.
(d) Mark sheets and pass certificates.
(e) Caste certificate (where applicable) from competent authority.
(f) Permanent Residential Certificate/Domicile Certificate from competent authority.
(g) Employment Exchange Registration Certificate.
(h) Diploma Certificate, wherever applicable.
